Case Management -Nurse Manager
Our client is a major medical center in Nor! thern NJ,
part of a prominent health system.
This is an exciting opportunity for a Nursing
Coordinator to be at the forefront of healthcare
changes, with the potential opportunity to partner with
our Accountable Care Organization. The goals are to
ensure continuous quality improvement is centered
around: Safety, effectiveness, patient-centered care,
timeliness, efficiency, and equitability.
Will function in a collaborative environment as a
resource to all Care Managers and Social Workers;
assisting with complex or difficult discharges;
supporting and facilitating management of Corporate
Compliance and policies relative to the Continuum of
Care; ensuring staff compliance with Department
policies; and participating in the evaluation and
completion of staff performance reviews.

• Our Care Management Coordinator position requires 5

years of diversified, progressive experience in acute care case management in a teaching environment and at
l! east 2 years of supervisory or progressive leadership
experience.

• Current NJ RN license and BSN degree required. Care

Management Certification preferred.

• Utilization experience ( including Milliman/

Interqual) and All Scripts knowledge preferred.

• Knowledge of Medicare/ Medicaid regulations required.

Hours 8-4 pm. Some weekends required as per department
policy.
