Licensed Practical Nurse - LPN: Telephonic Case Management

Location: Jersey City New Jersey

For complete informations about this career opportunity please read the description below. We are looking for Licensed Practical Nurses (LPN) for full time telephonic case management posi! tions.

This position is responsible for the following:

• Performing comprehensive assessment through communication with patient and healthcare provider to determine the degree of injury and disability, and the proposed treatment plan.
• Pre-certification of referred claims auto insurance carriers.
• Imputing the demographics, nature of claimant's injury, treatment plan and providers in the electronic medical record.
• Develop individual case management plans through assessments, and the planning and implementation of treatment plans.
• Establish and maintain contact with hospital staff and act as a liason to coordinate and identify goals for the injured party.
• Negotiate rates with out of network providers to provide the appropriate level of care and monitor cost savings.

This position requires the following:

• Must be licensed in the state of New Jersey
• Prior case managemen! t experience dealing with Workman's Compensation or PIP.

• Must be able to efficiently operate a computer including Word and Window NT
• Excellent verbal and written communication skills
• Must excel at problem solving and independent decision making

Responsibilities include:

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Performs focused interview to identify specific patients needs.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing di! agnoses.

Qualifications include:

• Current Licensed Practical/Vocational Nurse License.
• Full command, verbal and written, of the English language.
• Current Health Certificate (per facility).
• Current PPD or Chest X-Ray.
• One year prior professional nursing experience Preferred.
• Current BLS card.
• Successful completion of the screening tests
