Description: Partnership for Maternal and Child Health of Northern New Jersey is currently interviewing Home Visiting Nurse - Nurse Family Partnership Program right now, this position will be placed in New Jersey. This is an exciting opportunity to make a difference for first time mothers and their children by joining the Partnership for Maternal and Child Health of Northern New Jersey: Nurse-Family Partnership (NFP) program. The NFP is a nationally replicated nurse home visitation program that has been rigorously evaluated for more than 25 years. The program has demonstrated significant improvements in pregnancy outcomes, parenting skills and family self-sufficiency. Home visiting begins during pregnancy and lasts until the child's second birthday.As a member of the Nurse-Family Partnership (NFP) team you will provide prenatal and early childhood case management through regular home visits for first time mothers, carrying a caseload of no more than 25 families (per full time RN). A nurse home visitor (NHV) assesses the medical and social needs of high-risk pregnant women and infants; NHV’s follow a prescribed plan of care and monitor the outcomes. Qualifications : Registered Nurse (BSN or higher degree) with a minimum of two years nursing experience in OB/GYN, Pediatrics, Maternal Child Health, Community Health or Family Practice. Needs to be able to work flexible work schedule to include some evenings and occasional Saturdays.

Bilingual (Spanish / English) applicants; enthusiastic and committed to community health and social change; must demonstrate respect for individuals and diversity; be nonjudgmental, honest, understanding and compassionate. Applicant has strong advocacy and teaching skills, has organizational and time management skills, is able to utilize nursing processes and clinical expertise; builds therapeutic relationships, be able to work with a team of health care professionals. Applicant must be able to work in low-income communities; be able to travel to home visits by personal car. Independently motivated as well as a team worker. Must have a valid driver’s license and personal vehicle with up to date registration & insurance. Excellent written and verbal communication skills needed as well as basic computer skills.
