JOB DESCRIPTION

The Coalition is seeking a Nurse Care Coordinator to coordinate the planning and implementation of a new grant funded initiative focusing on the reduction and risk of childhood obesity in Camden. Supported by the Campbell’s Foundation and in coordination with the Foundation’s multi-year Childhood Obesity and Hunger Program, this program will broadly focus on three primary components: 1) Group prenatal and well child visits in primary care offices in Camden using the Pregnancy & Parenting Partners (P3) model; 2) working with and engaging primary care providers, including school nurses, in educational opportunities and creative strategies targeting family nutrition and wellness; and 3) implementing a data collection and tracking system to collect BMI measures through schools. This individual will work directly with clinicians, clinic staff, patients, partnering organizations, Coalition staff, and the Campbell’s Foundation. This position will be an employee of Cooper Hospital and will report directly to the program’s Project Manager.

DUTIES & RESPONSIBILITIES

Specific responsibilities of this position include:
Coordinate planning activities of the project; work

directly with clinicians and staff of Depts. of OB/GYN and Pediatrics at Cooper

in defining patient criteria, referral procedures, structure and content of

group visits (ensuring adherence to NFP standards and guidelines), data

collection, and other process details

Provide self care education to participating women,

including measuring height, weight, blood pressure, temperature, vital signs, and

infant growth development metrics; provide individual support to patients as

needed

Assist with medical care group visits; review

progress forms with patients; facilitate sharing of common questions, learning,

discussion, and support within the group setting

Work directly with Coalition staff in providing

family nutrition and wellness education to primary care providers and staff;

work with clinics and practices to implement innovative pilot strategies to

encourage healthy eating and food choices

Train school nurses to collect and input BMI data

into web-based tracking system

Assist with project data collection and evaluation

efforts

Other duties as required

QUALIFICATIONS & REQUIREMENTS

Bachelor’s Degree in Nursing required; Master’s Degree in Nursing or closely related field preferred; Spanish language ability a

plus

Experience in maternal/child health services and working in urban communities preferred

Current NJ nursing license (in good standing) or ability to obtain NJ licensure within six months of hire

Superior organizational and interpersonal skills, with attention to detail required; strong oral/written communication skills is a must

Candidates must have proven ability to work collaboratively and productively with clinicians, administrators, patients, and other individuals from various backgrounds, skill sets, and literacy levels
