We offer eight convenient locations to provide easier access to the care you need.Position SummaryUnder the direction of the Call Center Manager, the Triage Nurse ( RN ), will handle phone calls from the patients who are seeking medical information and/or medical attention. They will be able to schedule appointments as the patients condition requires, answer questions, and provide health education in relation to the call. They will be able to educate the patients on the phone who have medical questions. This position brings all the skills and experienced nurse into a telephonic environment. The Triage Nurse, utilizes the nursing process, motivational interviewing, critical thinking and clinical knowledge to identify health risks, develop appropriate action plans, and e! ngage the patient in implementing a plan to appropriately util! ize primary care and reduce health risks. Job Responsibilities Provide patient teaching, health promotion and disease prevention methods related to routine healthcare and those designed to address the needs of patients with chronic disabling health conditions. Screen and respond to patient phone calls, directing to primary care provider as needed. Schedule appointments, based on necessity, by offering the earliest appointment available with each provider. Develop appropriate action plans, and engage the patient in implementing a plan to appropriately utilize primary care and reduce health risks. Set priorities and anticipate primary care provider needs. Deliver medical advice per approved triage policy and process. Schedule urgent visits as appropriate. Accurately documents the encounter with the patient in Centricity for continuity of care. Enters necessary information into the computer. Initiate appropriate follow-up Demonstrate the ability to communicate effectively with! all members of the healthcare team, patients, and community representatives. Maintain confidentiality of all patient information. Adheres to policies/safety practices established by CAMcare Health Corporation. Performs other duties as assigned. RN - Registered Nurse Call Center Triage Nurse QualificationsEducation and Experience Bachelors degree in Nursing Graduate from an accredited RN Program Two Years of hospital or public health nursing experience Two Years nursing experience in medical center or community health care settings Additional Requirements At least one years of experience in each of the following disciplines: Internal Medicine, Pediatrics, OB/GYN. Knowledge of practical nursing theory, standard practices, rules, and regulations related to nursing. Familiarity with local community resources for patients with chronic disease. Familiarity with Patient-Centered Medical Home conc! epts. Ability to interpret communication styles of team members, patien! ts and contractors who come from a broad spectrum of disciplines. Strong organizational skills and multitasking abilities. Strong written and oral communication skills. Strong customer service skills. Must be able to learn and understand and apply new technologies. Ability to effectively prioritize and execute tasks in a high-pressure environment is crucial.
